Parking Agreement*

I understand that vendor parking is located at the tennis courts and a shuttle will be provided for me to use. I agree that I will adhere to the parking requirements and not take up the parking spaces in front of the school that are designated for fair customers.

Cancellation Policy*

I agree that Two (2) weeks’ notice MUST be given to receive a full refund. Less than two (2) weeks, no refund will be provided.

Liability Requirement*

I understand that neither the Friends of the Bath Youth Meetinghouse & Skatepark nor the Bath Middle School are liable for any damages, thefts, or injury.
